What Is a Cervical Herniated Disc?

A cervical herniated disc occurs when disc material between the vertebrae in the neck extends beyond its normal boundaries and can affect a nearby nerve root or, less commonly, the spinal cord.

The cervical spine contains seven vertebrae. Intervertebral discs contribute to load distribution and allow movement between vertebrae.

When the outer portion of a disc weakens or tears, inner disc material can extend outward. However, the presence of a disc protrusion on imaging does not automatically mean that it is responsible for the patient's symptoms.

Not every episode of neck pain is caused by a herniated disc. Muscular, joint and degenerative conditions can also cause neck pain.

What Causes a Cervical Herniated Disc?

Age-related degeneration of the intervertebral discs is one of the main mechanisms.

Discs can gradually lose water content, height and flexibility. Bone spurs can also develop and contribute to narrowing around the exiting nerve roots.

In younger adults, disc herniation may sometimes follow lifting, pulling, bending, twisting or trauma, although a specific triggering event is not always present.

What Are the Risk Factors?

Factors that can affect cervical disc and spine health include age-related degeneration, repetitive heavy loading, trauma, smoking, prolonged non-neutral working positions and repeated mechanical strain.

Poor posture can contribute to mechanical neck discomfort, but it should not be presented as the sole cause of cervical disc herniation.

What Are the Symptoms?

Symptoms depend on whether a nerve root or the spinal cord is affected.

  • Neck pain
  • Pain radiating into the shoulder, arm or hand
  • Numbness or tingling
  • Weakness in specific muscle groups
  • Changes in reflexes
  • Arm pain aggravated by certain neck movements

What Is Cervical Radiculopathy?

Cervical radiculopathy occurs when a nerve root in the neck is compressed or irritated.

A herniated disc is one possible cause. Degenerative bone spurs and narrowing of the neural foramen can also compress a nerve root.

Symptoms can include pain radiating from the neck into the shoulder, arm or hand, together with numbness, tingling, weakness or altered reflexes.

What Is Cervical Myelopathy?

A disc herniation or associated cervical stenosis can sometimes compress the spinal cord.

Neurological dysfunction resulting from spinal cord compression is called cervical myelopathy.

Possible features include gait imbalance, leg stiffness or weakness, loss of hand dexterity, difficulty with buttons or handwriting, numbness and, in advanced cases, changes in bladder or bowel control.

Progressive balance or walking problems, weakness in the arms or legs and declining hand dexterity should not be dismissed as routine neck pain. They can indicate spinal cord compression.

Which Symptoms Need Prompt Assessment?

  • New or progressive arm or hand weakness
  • Gait or balance disturbance
  • Progressive loss of hand dexterity
  • Leg weakness or stiffness
  • New bladder or bowel dysfunction
  • Neck and neurological symptoms after significant trauma
  • Severe neck pain associated with fever or systemic illness

How Is a Cervical Herniated Disc Diagnosed?

Diagnosis should not rely on MRI findings alone.

Assessment begins with symptoms and neurological examination, including muscle strength, sensation, reflexes, neck movement, gait and signs that might indicate spinal cord involvement.

When imaging is appropriate, cervical MRI provides detailed information about discs, nerve roots, the spinal cord and the spinal canal.

Is MRI Needed for Every Episode of Neck Pain?

No.

Imaging depends on the duration and pattern of symptoms, the presence of radiculopathy or neurological deficits, trauma, red flags and response to initial treatment.

For chronic cervical pain with radiculopathy, non-contrast cervical MRI is an appropriate primary imaging option.

How Is a Cervical Herniated Disc Treated?

Treatment should be based on symptoms and neurological findings rather than the size of the disc herniation on MRI alone.

Many people with cervical radiculopathy improve without surgery.

Treatment can include activity modification, medication, physiotherapy and exercise, selected interventional pain procedures and surgery when clinically indicated.

Nonsurgical Treatment

When there is no progressive neurological deficit, initial treatment can include appropriate analgesic or anti-inflammatory medication, physiotherapy, ergonomic modification, controlled exercises and gradual return to normal activity.

Prolonged complete bed rest is usually unnecessary.

When Is Surgery Needed?

Most cervical disc herniations do not automatically require surgery.

Surgery can be considered when radicular pain remains disabling despite appropriate nonsurgical treatment, when weakness progresses, when significant neurological loss develops or when spinal cord compression causes myelopathy.

The main surgical objective is to decompress the affected nerve root or spinal cord.

Which Operations Are Used?

There is no single operation that is the universal standard for every cervical disc herniation.

Depending on the anatomy, procedures can include:

  • Anterior cervical discectomy and fusion (ACDF)
  • Cervical artificial disc replacement
  • Posterior cervical foraminotomy
  • Minimally invasive or endoscopic decompression in selected patients

The choice depends on the site of compression, spinal alignment, stability and other patient-specific factors.

What Is Endoscopic Cervical Discectomy?

Endoscopic cervical discectomy is a minimally invasive technique in which an endoscope and specialised instruments are used through a small surgical corridor to decompress the affected nerve in selected patients.

It is not suitable for every cervical disc herniation.

Selection depends on the level and location of the herniation, the pattern of nerve-root or spinal-cord compression, associated stenosis, cervical alignment and spinal stability.

How Can Neck Health Be Supported?

Not every cervical disc herniation can be prevented, particularly age-related degenerative changes.

Helpful measures can include avoiding prolonged static positions, improving workstation ergonomics, positioning screens to reduce prolonged neck flexion, remaining physically active, using controlled lifting techniques and avoiding smoking.

Not Every Cervical Disc Herniation Requires Surgery

Treatment should be individualised according to pain duration, muscle strength, neurological findings and whether a nerve root or the spinal cord is compressed.

Is endoscopic cervical discectomy suitable for everyone?
No. Endoscopic cervical discectomy is used in selected patients. The level and location of the herniation, nerve-root or spinal-cord compression, stenosis, alignment and spinal stability must be considered.
