Enlargement of the breast area in men affects much more than aesthetics.
So, is the cause of this enlargement fat accumulation, glandular enlargement, or a hormonal disease?

Psychological Effects of Gynecomastia

This condition causes loss of self-confidence in many men. Avoidance of social environments such as the beach, gym, and tight clothing is common. Treatment provides not only physical but also psychological relief.

What Is Gynecomastia?

Gynecomastia is the excessive enlargement of the breast gland, glandular tissue, in men due to hormonal causes. There is a balance between testosterone and estrogen hormones in men. When this balance shifts in favor of estrogen, breast gland tissue may enlarge.

This condition:

  • May occur in one breast
  • May occur in both breasts
  • May cause tenderness and pain
  • May be felt as a hard mass

What Are the Types of Gynecomastia?

It is divided into different classes according to its appearance, cause, and the structure of the breast tissue. Correct classification means correct treatment.

  • True Gynecomastia (Glandular Type): Breast gland tissue enlarges. It is hard.
  • False Gynecomastia (Pseudogynecomastia): It is only fat accumulation. It is soft.
  • Mixed Type Gynecomastia: Both fat and glandular tissue have increased. It is the most common type.
  • Physiological Gynecomastia: It is related to hormonal changes during puberty or old age. Sometimes it resolves spontaneously.
  • Pathological Gynecomastia: It develops due to a disease, hormonal disorder, or medication. First, the underlying cause is investigated.

What Is False Gynecomastia (Pseudogynecomastia)?

False gynecomastia is the enlargement of the breast area in men caused by fat tissue rather than glandular tissue.

Why Does False Gynecomastia (Pseudogynecomastia) Occur?

  • Excess weight and increased body fat ratio
  • Sedentary lifestyle
  • Unhealthy nutrition
  • Long-term weight gain

In some men, fat tissue tends to be stored especially around the breast area. This is entirely related to the difference in genetic distribution.

How Are True Gynecomastia and False Gynecomastia Distinguished?

FEATURE TRUE GYNECOMASTIA FALSE GYNECOMASTIA
Hard disk under the nipple Present Absent
Tenderness when touched May be present Usually absent
Reduction with weight loss Little Significant
Hardness when pressed by hand Felt Soft

How Is Gynecomastia Diagnosed?

Gynecomastia is diagnosed primarily by physical examination. Ultrasonography is performed if necessary. Hormone tests are used to investigate whether there is an underlying hormonal problem. In rare cases, mammography or MRI may be requested. Although male breast cancer is rare, it should be ruled out in risky findings. It is necessary to distinguish whether breast enlargement in men is fat accumulation or true breast gland increase. This distinction directly affects the treatment plan.

Frequently Asked Questions

Does Gynecomastia Recur?
If the underlying cause continues, for example steroid use, it may recur. However, the recurrence rate is low after surgery performed with the appropriate surgical technique.

Does Gynecomastia Resolve on Its Own?
Cases seen during puberty can often regress within 1–2 years. In adulthood, it is permanent and usually requires surgery.

When Can Sports Be Done After Surgery?
Light exercises can be started after 3–4 weeks. For weight training, usually 6 weeks are waited.
