What Is Midface Filler?

Midface filler is a nonsurgical aesthetic treatment used to restore or support volume around the cheeks and cheekbones and to improve facial contour in appropriately selected patients.

One of the most commonly used materials is hyaluronic acid dermal filler. Hyaluronic acid occurs naturally in the body, while injectable aesthetic fillers are specially manufactured medical products with specific physical properties.

The aim is not to completely change the face or return a person to a particular age, but to support lost volume while respecting the individual's existing facial anatomy.

How Does the Midface Change With Age?

Facial ageing involves more than changes in the skin.

Over time, changes can occur in facial fat compartments, connective tissues, supporting structures and facial bones. These changes may contribute to flattening of the cheeks, a more visible transition between the lower eyelid and cheek and greater prominence of the nasolabial folds.

Midface filler cannot correct every feature of facial ageing. Significant skin laxity, true under-eye bags or other anatomical concerns may require different treatment approaches.

Who May Not Be Suitable?

A detailed medical history should be reviewed before dermal filler treatment.

Treatment may need to be delayed or avoided in people with active infection or inflammation at the treatment site, known serious hypersensitivity to filler components, relevant bleeding disorders or medications, or a history of serious filler-related complications.

Because safety data are limited during pregnancy and breastfeeding, elective filler treatment is generally deferred during these periods.

How Is Midface Filler Performed?

Before treatment, facial anatomy and the pattern of volume loss are assessed. Injection points, depth and the amount of filler are planned individually.

A topical anaesthetic may be used when appropriate. Hyaluronic acid filler can then be placed into the selected anatomical plane using a needle or cannula.

Procedure time varies according to the treatment area and technique, so a fixed duration such as 40 minutes does not apply to every patient.

How Long Does Midface Filler Last?

Hyaluronic acid fillers are temporary.

Duration depends on the product used, treatment area, injection depth, amount injected and individual anatomical and metabolic factors.

It is therefore not accurate to promise that every midface filler will last exactly 12 or 18 months. The effect gradually decreases over time and may persist longer with some products and treatment plans.

What Are the Side Effects and Risks?

Dermal filler injection is a medical procedure and is not completely risk-free.

More common and usually temporary effects include:

  • Swelling
  • Bruising
  • Redness
  • Tenderness or pain
  • Temporary asymmetry
  • Small palpable irregularities

Less common complications include infection, nodules, granulomas, inflammatory reactions and unwanted movement of filler material.

Rare but serious risk: vascular occlusion

If filler unintentionally enters or compromises a blood vessel, reduced blood flow and tissue injury can occur. Rare serious complications reported with facial filler injections include visual impairment, blindness and stroke.

Severe unusual pain, whitening or mottled discoloration of the skin, sudden visual change, double vision or neurological symptoms during or after treatment require urgent medical assessment.

What Should Be Considered After Treatment?

Mild swelling, redness, tenderness or bruising can occur after treatment and usually improves over time, although recovery varies between individuals.

Patients should follow the specific aftercare instructions provided by the treating clinician and avoid unnecessary pressure or manipulation of the treated area unless advised otherwise.

Increasing pain, marked skin colour change, excessive swelling or any visual symptoms require prompt medical assessment.
