Exosomes and other extracellular vesicle-based products are an emerging area of research in regenerative and aesthetic dermatology.

Early clinical studies have reported potentially promising results. However, exosome source, manufacturing, purification, concentration and delivery methods vary substantially between studies and commercial preparations.

What Are Exosomes?

Exosomes are a group of very small membrane-bound extracellular vesicles released by cells.

They can participate in intercellular signalling by transporting biological molecules between cells.

Depending on their cellular source and manufacturing method, their cargo can include proteins, lipids and different types of RNA.

Are Exosomes Stem Cells?

No.

Exosomes are not living cells and are not stem cells.

Many types of cells, including stem and stromal cells, release extracellular vesicles.

Therefore, describing an exosome product as containing stem cells or being equivalent to stem-cell therapy is not technically accurate.

Exosomes are not stem cells. They are small extracellular vesicular structures that can carry biological molecules released by cells.

What Can Exosomes Contain?

Exosome cargo is not identical across preparations.

Depending on the source cell and biological conditions, extracellular vesicles can carry:

  • Proteins
  • Lipids
  • mRNA, microRNA and other RNA species
  • Other signalling molecules

A commercial product labelled as containing exosomes should not automatically be assumed to have the same concentration, purity or biological effects as preparations evaluated in published research.

What Are Exosome-Based Skin Applications?

In aesthetic dermatology, extracellular vesicle-containing preparations are being studied as potential adjuncts for improving skin quality.

Human studies have evaluated topical application as well as their use after procedures such as:

  • Microneedling
  • Fractional laser procedures
  • Other energy-based skin treatments

These procedures can temporarily alter the skin barrier and potentially increase penetration of topical preparations. However, treatment protocols remain heterogeneous.

What Does the Clinical Evidence Show?

Human clinical evidence is increasing, but exosome-based aesthetic treatments do not yet have the depth of large, standardised and long-term clinical evidence available for established aesthetic therapies.

A 2026 systematic review of human studies reported short-term associations with improvements in:

  • Skin hydration
  • Elasticity
  • Wrinkle appearance
  • Pore appearance
  • Pigmentation
  • Overall skin appearance

However, most included studies were not randomised and there was substantial variation in products, treatment methods and follow-up periods.

Claims such as “guaranteed rejuvenation”, “guaranteed collagen production” or “complete acne-scar removal” are not supported by the current evidence. Clinical findings are encouraging but remain preliminary and heterogeneous.

Which Effects Are Being Studied?

Laboratory, animal and early human research is investigating potential effects on:

  • Fibroblast activity and extracellular matrix regulation
  • Collagen metabolism
  • Skin hydration and barrier function
  • Oxidative stress pathways
  • Inflammatory signalling
  • Wound repair
  • UV-related skin ageing
  • Pigmentation

Results from cell-culture or animal studies cannot automatically be assumed to produce the same clinical effect in human skin.

Exosomes and Microneedling

One approach evaluated in aesthetic studies combines microneedling with a topical extracellular vesicle-containing preparation.

Microneedling creates controlled microchannels within the skin. In some protocols, the preparation is applied topically after the procedure.

Because microneedling itself can improve several skin parameters, it may be difficult to determine how much of an observed improvement is attributable specifically to the exosome-containing preparation.

What Does the Procedure Involve?

There is no single standard procedure for every exosome product.

When combined with microneedling, a typical clinical process may include:

  1. Assessment: Skin type, medical history, current skin conditions, medicines and treatment goals are reviewed.
  2. Preparation: The skin is cleansed and topical anaesthesia may be used depending on the procedure.
  3. Microneedling: Controlled microchannels are created.
  4. Topical application: The selected preparation may be applied according to the treatment protocol.
  5. Aftercare: Skin-barrier care and sun protection are discussed.

Procedure duration depends on the treatment area and accompanying technique rather than having one fixed time for all patients.

How Many Sessions Are Needed?

There is currently no universally established evidence-based number of sessions for all exosome-containing products.

Published studies use different dosing schedules and treatment intervals.

The clinical plan may therefore depend on the preparation, accompanying procedure, skin characteristics, treatment target and clinical response.

Aftercare

Aftercare is largely determined by any accompanying microneedling or laser treatment.

Recommendations can include sun protection, appropriate moisturisation and barrier support, temporarily avoiding irritating skin-care products and following the clinician's individual instructions.

Potential Side Effects and Risks

When combined with microneedling or laser treatment, temporary effects can include:

  • Redness
  • Tenderness
  • Stinging or burning
  • Swelling
  • Dryness or peeling

Some of these effects may be caused by the accompanying procedure rather than the extracellular vesicle preparation itself.

With biologically derived products, manufacturing quality, purity, sterility, source material and standardisation are important safety considerations.

Injection of an exosome-labelled product into the skin is not equivalent to topical application. Route of administration is an important safety consideration, particularly when product composition or standardisation is uncertain.

Are All Exosome Products the Same?

No.

Preparations may differ substantially in:

  • Biological source
  • Manufacturing process
  • Isolation and purification
  • Vesicle concentration
  • Biological cargo
  • Sterility and quality control
  • Storage conditions

This variability makes it difficult to extrapolate results obtained with one research preparation directly to another commercial product.

The word “exosome” on a product label does not by itself demonstrate efficacy or safety. Product source, quality control, sterility and regulatory status should be considered before use.

Who May Be a Candidate?

Universal patient-selection criteria have not yet been established.

Existing aesthetic studies have mainly investigated skin texture and quality, photoageing, fine lines, pigmentation irregularities and recovery following microneedling or laser procedures.

Suitability should be determined after individual medical and skin assessment.

When May Treatment Be Postponed?

Especially when microneedling or laser is involved, treatment may need to be postponed in the presence of:

  • Active infection at the treatment site
  • Active herpes infection
  • Uncontrolled inflammatory skin disease
  • Significant wound-healing problems
  • Important medical conditions or treatments affecting immune function

Pregnancy, breastfeeding, autoimmune disease, cancer history and regular medication use should be discussed individually because safety data for several patient groups remain limited.
